Digital & Tech Social Prescribing Link Worker (N. Cotswolds)

In partnership with the NHS North Cotswolds Patient Care Network (PCN) , our Digital and Technology Social Prescribing Link Worker (SPLW) plays a vital role in supporting vulnerable individuals across a very rural area.

Working closely with five GP surgeries and outreach clinicians, this specialist role is designed to reduce digital exclusion among patients whose health may be poor or declining — and for whom digital support can make a significant difference.

Grounded in the Social Prescribing model, this highly innovative role offers digital support as a fundamental aspect of care.

The key focus of this role is on home visits, which are essential for those who find it difficult or impossible to leave their homes.

Support provided during home visits includes:

  • Helping patients access and confidently use the NHS App
  • Setting up emergency alarm call buttons and other home safety devices
  • Improving tech device security and increasing awareness of scams
  • Introducing communication tools to reduce isolation and connect individuals with online communities, such as long COVID support groups
  • Installing and explaining GP-recommended apps, such as those for tracking blood pressure
  • Setting up smart technology like Google Home or Alexa, to support independent living - for example, voice-activated lights or hands-free telephone calling

By bringing technology into patients’ homes, we aim to empower individuals, improve health outcomes, and ensure no one is left behind in an increasingly digital world.
